Suppose you are a non-EU national living in the Czech Republic and your long-term visa, long-term residence permit, or permanent residence permit has been lost, stolen, or damaged. In that case, you are required to report it to the Czech authorities.
Time Limit for Reporting
You must report the loss, theft, or damage of your visa or residence document within 3 working days. Delays in reporting may lead to a fine.
Who You Need to Notify
In case of loss or theft, it is mandatory to report the incident to the police as soon as possible. If the document is lost abroad, you should report it to the Czech embassy or consulate in the respective country.
Where to Report the Loss or Damage
The situation can be reported to the Ministry of the Interior of the Czech Republic. You can do so in person, by post, via Data Box, by email with a recognized electronic signature, or through an authorized representative.
What Happens if the Lost or Stolen Document Is Returned to You
If you recover your document after having reported it as lost or stolen, you must inform the Ministry of the Interior. The procedure for doing so is the same as when initially reporting the incident.
Administrative Fees

Visa duplicate: No administrative fee.

Replacement residence permit card: 2,500 CZK (1,000 CZK for minors under 15 years of age).
The fee can be paid via bank transfer or by card directly at the office.

Once the loss is reported, additional steps may be required before a replacement document is issued. These may include providing a new travel document, filling in the notification form, biometric data collection, and scheduling an appointment with the Ministry.
